To take advantage of the SBHC services, please use the links below to access the required forms (if possible, print packets double-sided):
OPTIONAL: Families have the option to permit the SBHC therapist and BHS school staff to communicate with each other. Please complete this form if you wish to allow for communication between BHS school staff and the SBHC provider. If chosen, the optional form should be returned directly to Samantha Mei, Licensed Clinical Social Worker, by email.
